As a new patient, we advise that you arrive 30 minutes prior to your appointment. This is so that our front office is able to verify your information and make sure you fill out all necessary paperwork prior to your appointment. This is to better serve our patients!

If you want to be one step ahead of the office, fill out the new patient paperwork forms found below and bring them with you.
Conner Family Health Care believes that health care should be affordable. We offer two pays patients without insurance to pay for their visits.
- We offer a membership plan for individuals and families. Please ask the front office for more information regarding membership plans.
- We also allow you to pay per visit. The first visit is $145 and every subsequent regular visit is $118.
We understand that our patients are busy people with busy lives. That is why we allow for patients to reschedule until 24 hours before their appointment. If you try to reschedule the day of, or if you no show, we will charge you $45 for the appointment.
